- These services are provided for wellness, lifestyle, and elective purposes and are not a substitute for care by a primary care physician or specialist.
- Many of our treatments have not been evaluated or approved by the U.S. Food and Drug Administration (FDA) for the purposes we use them.
- By receiving these services, you understand and accept their alternative and non-FDA-approved nature.
Informed Consent & Patient Autonomy
You acknowledge that:
- You are choosing to receive these services voluntarily.
- You have had the opportunity to ask questions and receive full information.
You understand the risks, limitations, and alternative options, including standard medical care. - You may discontinue treatment at any time.
Cash-Pay Agreement
- Revive Infusions and Wellness is a cash-pay practice.
- We do not bill or participate in any insurance programs, including:
- Medicare
- Medicaid (OHP)
- TRICARE
- Commercial health plans
- You are fully responsible for all charges and agree not to submit claims for reimbursement unless allowed under your private plan.
Telehealth Consent (if applicable)
By participating in virtual visits, you acknowledge:
- The provider is licensed in Oregon, and services are provided only to residents located in Oregon at the time of the visit.
- Telehealth carries limitations, including the inability to perform a full physical examination.
- In the event of an emergency, you agree to contact 911 or go to the nearest ER.

Patient Portal Privacy & Security Policy
At Revive Infusions & Wellness, we are committed to protecting your privacy and ensuring the secure handling of your health information. This policy outlines how our patient portal complies with the Health Insurance Portability and Accountability Act (HIPAA) and Oregon state law.
1. Portal Access
Access to the patient portal is available only to active, verified patients of Revive Infusions & Wellness. Each portal account is securely linked to a clinical record and must be activated directly by our team.
2. Security Measures
Our EMR system uses multiple safeguards to protect your information, including:
- End-to-end encryption on all portal communications
- Role-based access controls so only authorized users can view sensitive data
- Secure login with username and password (with optional two-factor authentication)
3. Limitations on Demonstrations
Due to the privacy features of our EMR system, we cannot create test or demo patient profiles from the patient view. For this reason, we are unable to provide screenshots or third-party access to the portal without exposing Protected Health Information (PHI) or Personally Identifiable Information (PII).
4. No Third-Party Portal Access
Revive Infusions & Wellness does not allow access to the patient portal for any third parties, including certifying bodies. All platform reviews are conducted through staff-side documentation and written explanations only.
5. Data Retention & Confidentiality
We retain patient health records for a minimum of seven (7) years, or longer in the case of minors, in accordance with Oregon law. Your information is never sold, shared, or used outside of your care unless required by law or authorized by you in writing.
