REVIVE Infusions & Wellness offers telehealth (telemedicine) services to patients located in Oregon, Washington, and Florida. This disclosure explains what telehealth is, how it works at REVIVE, and what you should know before scheduling a virtual visit.

What Is Telehealth?

Telehealth allows you to receive medical consultations, follow-ups, and certain treatment guidance remotely through secure video or phone communication, rather than an in-person office visit. At REVIVE, telehealth is used to support services such as hormone optimization, peptide therapy consultations, medical weight loss follow-ups, and general functional medicine check-ins, where clinically appropriate.

How It Works

  • Telehealth visits are conducted through a secure, HIPAA-compliant video platform.
  • You will need a reliable internet connection, a device with a camera and microphone, and a private space for your appointment.
  • Your provider will review your history, discuss your goals and symptoms, and determine whether telehealth is appropriate for your care or whether an in-person visit, lab work, or additional evaluation is needed.
  • Prescriptions, when clinically appropriate, may be sent electronically to a pharmacy of your choice, subject to state and federal regulations.

Who Can Use Telehealth at REVIVE

Telehealth services are currently available to patients physically located in Oregon, Washington, or Florida at the time of their appointment, consistent with our providers’ state licensure. If you are located outside these states, we may not be able to provide telehealth services to you, and we will let you know if an in-person visit or referral is more appropriate.

Benefits and Limitations of Telehealth

Benefits may include increased convenience, reduced travel time, and easier access to follow-up care.

Limitations may include:

  • The inability to perform a hands-on physical examination
  • The possibility that your provider may determine an in-person visit, lab testing, or imaging is necessary to continue your care safely
  • Technical issues (such as connectivity problems) that could interrupt or delay a visit

Your provider will always let you know if telehealth is not suitable for your specific situation and will help direct you to appropriate in-person care.

Privacy and Security

Telehealth visits are conducted using platforms designed to meet HIPAA security standards. As with any technology, we cannot guarantee that electronic communications are completely free of risk, but we take reasonable steps to protect the confidentiality of your information during and after your visit. For more detail on how your health information is used and protected, please see our Notice of Privacy Practices.

Insurance and Billing

Telehealth visits may be billed differently than in-person visits depending on your insurance plan. Many consultations and follow-ups, including those related to hormone optimization or peptide therapy, may be reimbursable through out-of-network benefits even when the underlying treatment isn’t covered by insurance directly. We recommend confirming your specific telehealth coverage with your insurance provider prior to your appointment.

Consent

By scheduling and participating in a telehealth appointment with REVIVE, you acknowledge that:

  • You understand the nature of telehealth services and their benefits and limitations as described above.
  • You consent to receive care via telehealth, understanding that your provider may recommend in-person follow-up at any time.
  • You confirm you are physically located in Oregon, Washington, or Florida at the time of your appointment.
